Dietary Intervention in Acute Liver Dysfunction: A Case Study

Dietary history leading up to presentation included irregular meal timings, frequent oily and spicy food, late and heavy dinners, excess tea, coffee, and carbonated beverages, and low intake of fruits and light vegetables.

Baseline lab findings:

  • ALT: 301 U/L

  • AST: 277 U/L

  • GGT: 490 U/L

  • Total Bilirubin: 3.53 mg/dL

  • CRP: 15.69 mg/L

  • ESR: 34 mm/hr

These findings indicated acute hepatic inflammation with a systemic inflammatory response.

Approach

A structured 14-day dietary and lifestyle protocol was implemented, built around three Ayurvedic principles: Deepana–Pachana (enhancing digestion), Ama reduction (clearing metabolic toxins), and Rakta Prasadana (supporting healthy blood quality).

Dietary principles:

  • Light, warm, and easily digestible foods (Laghu Ahara)

  • Reduced heavy, oily, and processed foods

  • Mild digestive spices

  • Emphasis on hydration and natural fluids

  • Regular meal timing to support metabolic rhythm

Daily routine, over 14 days:

  • Early morning (6–7 AM): Cardamom–coriander infused water (½ tsp spices boiled in 500ml water, reduced to ~150ml)

  • Breakfast (8 AM): Light preparations — rice porridge, neer dosa, or upma

  • Mid-morning (11 AM): Fresh juices — lime-mint, bitter gourd, or diluted fruit juice

  • Lunch (1 PM, main meal): Rice or roti with lentils, cooked vegetables, and a small quantity of ghee

  • Evening (4 PM): Fresh fruits, eaten separately

  • Dinner (7 PM): Light meals — porridge, soups, or soft rice preparations

  • Bedtime (9 PM): Soaked raisins with water

Foods avoided: fried and oily foods, processed and refined foods, heavy dairy and red meat, excess tea, coffee, and alcohol, fermented mil products,carbonated drinks, and spicy, sour, or salty foods.

Outcome

Post-intervention lab findings (13 days later):

  • ALT: 36 U/L

  • AST: 19 U/L

  • GGT: 94 U/L

  • Total Bilirubin: 0.71 mg/dL

  • CRP: ~2.4 mg/L

  • ESR: 11 mm/hr

Liver enzymes normalized, bilirubin dropped sharply, and inflammatory markers decreased significantly. Appetite and sleep both improved alongside the biochemical recovery.

Notably, GGT — typically one of the last enzymes to normalize during recovery — remained mildly elevated even as ALT, AST, and bilirubin returned to baseline. This is generally understood as a sign of an ongoing recovery phase rather than active liver damage, reflecting continued healing of hepatic and biliary pathways. From an Ayurvedic perspective, this phase reflects residual Pitta and Ama clearance from the Yakrit — metabolic balance improving, but not yet fully stabilized.

Discussion

This case suggests that a structured dietary intervention can meaningfully improve liver function within a short window. The rapid normalization of biochemical markers suggests the underlying dysfunction was largely functional and reversible rather than structural.
